Inside the caves were huddled a filthy mass of wet, battered people.

Fred and the rest get down to helping them.
It's a mass hallucination caused by the fungi in the caves and the pee fumes.
The people seem to be ghosts. They are interacting with a 'ghost' version of the island. Fred and the others aren't seen.
Actually, they were shapechanging Chorlaths that Fred had trained to be humanlike and dance for his amusement.
